Eligibility      Other Services under this Program:           Community— Case Management Communication Devices Consumer Directed Attendant Option (Age 18 and older) Day Services Family/Individual Support Home Modifications Personal Emergency Response Residential Services Respite Care Sleep Cycle Support Van lift Wheelchair Modifications Health— Wellness Monitoring Persons with Physical Disabilities Waiver This Waiver serves people with physical disabilities and/or those aged 65 and older who need support to do daily activities. These services allow people to live at home and in their community.
