Eligibility      Other Services under this Program:           Community— Adult Day Care Assistive Technology Attendant Care Case Management Medication Reminder System Personal Emergency Response Sleep Cycle Support Health— Nursing Evaluation Visit Wellness Monitoring Persons with Physical Disabilities Waiver This Waiver serves people with physical disabilities and/or those aged 65 and older who need support to do daily activities. These services allow people to live at home and in their community.
