There are 964,300 people with a disability using Medicaid in California. The state spent $14,437 on each Medicaid recipient with a disability in 2007.

9% of the people covered by Medicaid in California have a disability, which is lower than the national percentage of 15%.

California spends 42% of all the money it spends on Medicaid on services for people with disabilities, though, which is the same as the national percentage.

California Basic Medicaid Programs

The State of CaliforniaCalOptima (Orange County)

CalOptima is a public health program organized and run by Orange County. This program provides adults and children with disabilities access to a variety of basic medical services. You may not get to choose your service provider under this plan.

Restrictions: This plan is limited to Orange County.

This program provides specialty treatment to California residents from all counties with certain mental health diagnoses including developmental disorders (excluding autism) that cause or might cause trouble in one or more areas of daily living. Partnership Health Plan of California (Marin and Mendocino Counties)

This plan provides adults and children with disabilities with access to basic medical services. You may not always get to choose your provider under this plan.

Two-Plan Model

This plan is implemented county by county in twelve counties in California. It was designed to expand Medicaid coverage to people who were not being served by existing county or local plans, including many adults and children with disabilities and some who are also eligible for Medicare. It does not cover people who are covered under an HCBS waiver or those residing in nursing or intermediate care facilities.

Restrictions: This plan is limited to Alameda, Contra Costa, Fresno, Kern, Los Angeles, Riverside, San Bernardino, San Francisco, San Joaquin, Santa Clara, Stainslaus, and Tulare counties.

Prepaid Health Plan Program

This program provides comprehensive benefits to adults and children with disabilities eligible for California Medicaid as an alternative to plans where you pay each time you use a service. People receiving in-home care, those participating in the HCBS waiver or a nursing facility waiver, and some who are also eligible for Medicare are not covered.

Senior Care Action Network (SCAN) (Riverside, San Bernardino, and Los Angeles Counties)

The SCAN program is a non-profit health care plan designed to meet the needs of older adults, including those with disabilities. To qualify for this plan, applicants must be at least 65 years old and qualify for both Medicare and Medicaid.

Ages: 65 and older

Restrictions: You must be Medicaid and Medicare dual eligible. This plan is limited to Riverside, San Bernardino, and Los Angeles counties.

Program of All-Inclusive Care for the Elderly (PACE)

PACE programs are designed for people 55 and older whose needs might require nursing home placement, which might include many people with disabilities. These programs provide care and support including a high level of preventive care. PACE programs must provide all services otherwise included in a person’s Medicaid or Medicare benefit, as well as all other services deemed medically necessary. Services are provided through PACE centers, so people must live close enough to an approved center to be eligible.

Ages: 55 and older

Restrictions: You must have access to an approved PACE center.

California HCBS Waiver Programs

The State of CaliforniaCalifornia Assisted Living Waiver

This waiver is available to people with disabilities living in certain counties. This waiver is designed to help people get needed services while remaining in their own homes and communities. Through this program, people with disabilities are eligible to receive assisted living (in a residential facility) or assisted care (in the community or home) services.

Level of Care: nursing

Options for Self-Direction: This program will provide education to help people with disabilities take control of their own care.

Restrictions: This plan is limited to San Joaquin, Los Angeles, Sacramento, Fresno, Riverside, and San Bernardino counties.

  • Community Community: transportation, recreational and social services, transition services, education
  • Health Health: skilled nursing, help with medications, facility maintenance
  • Home Home: personal care, laundry, housekeeping, meals and snacks

Developmentally Disabled/Continuous Nursing Care (DD/CNC) waiver

This waiver currently provides services to a small number of California residents in seven locations throughout the state. It is designed to help people with disabilities and others who need high levels of care remain in the community and avoid placement in nursing homes or other institutions.

Level of Care: nursing

Restrictions: You must live in a service area. Contact your local Medicaid providers for more information.

  • Health Health: 24-hour nursing, nursing supervision, ventilators and oxygen delivery, IV therapies, tube feeding if needed, medication assistance, and other skilled nursing services
  • Home Home: personal care, assistance with ADLs and IADLs

In-home Operations Waiver

This waiver is available to people with physical disabilities, no matter their age, who would otherwise need to be placed in a nursing facility. The waiver is designed to help them avoid placement in an institution and receive care at home.

Level of Care: nursing

Restrictions: This waiver has only 210 slots, so there may be a waiting list for services.

Pediatric Palliative Care Waiver

This waiver provides end-of-life care for children who have life-threatening illnesses, while at the same time allowing their families to pursue treatment (so that families no longer have to choose only one of these options). This care can be provided at home instead of in a hospital or residential treatment facility.

Ages: Children under age 21

Restrictions: Children must have a life-threatening or life-limiting illness, and must be eligible for Medi-Cal. The family must live in one of these counties: Marin, Fresno, Humboldt, Alameda, Monterey, San Diego, Santa Clara, Santa Cruz, Orange, Sacramento, San Francisco, Los Angeles, and Sonoma.
